What a repair receipt should list
The vehicle and mileage in and out, each part with its price, labor hours and rate, shop supplies, tax (often on parts only), the total, payment method, the warranty and the customer's authorization.
Parts and labor taxed differently
Many states tax parts but not labor. The tax row here takes a fixed amount so you can tax just the parts; switch it to a rate if your state taxes the whole bill.

Do the totals add up by themselves?

No. A receipt for a real purchase comes from the business that took the payment. Use this template to write receipts for payments you received, and for props, design, training and fun. Making one up to claim money is fraud.
